Court Forms

Here are some Court Forms you may find useful. These forms may be used if an agreement cannot be reached using Mediation.

FM1 Form

This form is only to be used when making an application to the court on form C1 or C2 for one of the orders shown on the form.

 

 

 

COMPLETING THE FM1 FORM

Form FM1 is a court form in family proceedings dealing with the requirement for the applicant in family proceedings to attend a mediation information and assessment meeting (MIAM) prior to the issue of proceedings. The applicant must confirm whether they have attended a MIAM, and if not the reason why.

C100 Form

Use this form to apply for a 'child arrangements', 'prohibited steps' or 'specific issue' order under the Children Act 1989.

 

COMPLETING THE FORM C100

Once you have your legal MIAM certificate signed/ your exemption decided. You now complete the contact order form C100. 

 

 

 

 

Form E

Form E is the financial statement standard-form document used to set out each party's relevant financial and other circumstances when a financial application has been made.

 

COMPLETING THE FORM E

You and your spouse or civil partner must each fill out separate Form Es if you or your spouse or civil partner applies for a financial order or financial relief. You should only complete Form
E if you or your spouse or civil partner has made an application for a financial order (which you can only apply for as part of a divorce, dissolution, annulment or (judicial separation) in England and Wales) or if you or your spouse or civil partner have made an application for financial relief following an overseas divorce/dissolution etc.
